The Diabetes Program’s philosophy is to provide a standard of care that creates optimal diabetes health care for each individual within a comfortable and pleasant environment so that each visit is a positive experience.

Services Include:

  • Weekly diabetes clinic
  • Diabetes education
  • Monthly optometry clinic
  • Nutrition Education
  • Freezer Meal Workshops
  • Lifestyle Coaching
  • Pathways Case Management Program
  • Community Screenings
  • Diabetes Prevention Youth Services
  • Diabetes Prevention classes/HoChunk Hope (1 year)
  • Diabetes Self-Management
  • Education & Support Classes/ Wa I pi nagu (10 weeks)

Diabetes Clinic is held weekly on Thursdays from 9 a.m. – 5 p.m. Patients can expect an individualized Diabetes Care Plan with an integrated team. Services provide through this clinic include:

  • Diabetes medication review
  • Diabetes medication adjustments and prescription
  • Diabetes labs
  • Goal setting
  • Foot inspection and exam
  • Comprehensive examination
  • Follow-up appointments in accordance with the individual treatment plan
  • Coordinate specialties in accordance with treatment plan such as nutrition team, case management, exercise staff, classes or ongoing diabetes education visits
